The vertical format with an arched top is divided into two pictorial fields. The essential act takes place within a circle of clouds with winged angels' heads in the upper part of the panel. On the bottom half of the panel there is an ideal landscape with a river and castles. The donor kneels in the foreground on the left wearing a fur-trimmed coat and holding rosary beads while he prays. Behind him is his son. On the right both his wives are depicted in a similar attitude of prayer and between them the donor's daughter. Those members of the family who were deceased when the epitaph was executed are depicted with a cloth over their mouths. The Assumption and Coronation of the Virgin take place at the same time.

Epitaph for Els († 1514) and Hans Warmuth († 1525), depicted as donors with their family, their names and dates of death are recorded in the inscription below.
